I've used Hazel Olive's Simplicity Card Templates (5x7) and adapted one to my 12x12 page.

In addition, I have used a staple kit for me, Journey Back by the ever-talented Vinnie Pearce for all papers and elements except, the striped paper is from My Two Peas by the fun and funky designer, Lauren Reid and the background paper is from new to TwoPeas designer, Angie Kovacs' Autumn Blossom.
