METHOD:
I opened the Duo template PSD and switched all the layers off. After adding my photo and frame, I added the BG paper and put another paper over it and changed the blending mode to Pin Light to show the aged effect through. I gradually turned all layers back on, deleting some, moving some around, etc. I added the transfers and brushes along with the word art. With the outside frame, I used one of the Artsy Grams and enlarged it to cover the entire page. I then just deleted the square inside and roughed the edges up slightly.
Finally, I added the bit at the bottom. It's a transfer (no 3) which I rotated and duplicated and flipped. I deleted parts to make it look like a separate piece of paper.
